Requirements
Before a child can start in our program, we must have the following:
• Completed application
• A signed statement of our policies
• A form signed stating that parent/guardian has received the summary of approval requirements from the Tennessee Department of Education
• A health history form completed by parent/guardian
• Registration fee
• Activity fee
• The first and last week's fee
• A complete record of current immunizations
• Birth certificate copy
For preschool children 30 months and younger, parents must also submit:
• A health form signed by your child's physician
(The health form requirement is in compliance with the laws mandated by the Tennessee Department of Education.)
